Summary
Deliver a high standard of nursing care within a general practice setting, ensuring patient safety and well-being Provide high-quality, evidence-based care for patients with long-term conditions such as diabetes, asthma, COPD, hypertension, and cardiovascular disease. Support the management of long-term conditions (e.g. diabetes, asthma, COPD, hypertension) through routine monitoring and reviews Undertake a range of clinical procedures, including wound care, dressings, suture removal etc. Carry out immunisations and vaccinations in line with national schedules and guidelines. Provide health promotion advice on areas such as smoking cessation, weight management, diet, and exercise. Assist with infection prevention and control, including maintaining a clean and safe clinical environment. Manage and maintain clinical stock, including ordering and safe storage of vaccines and medications. Record accurate and up-to-date patient information using electronic health record systems. Work collaboratively with GPs and the wider multidisciplinary team to deliver coordinated care. Recognise and respond appropriately to safeguarding concerns for children and vulnerable adults. Participate in audits, quality improvement activities, and practice development initiatives. Maintain professional development and ensure practice is in line with current clinical guidelines and standards.
