Summary
Care Coordinator – Sheffield (S2)
Salary: Up to £33,500
Hours: 45 per week
Contract: Permanent
Juventa 4 Care Ltd is seeking an experienced and organised Care Coordinator to support the smooth delivery of our domiciliary care services in Sheffield.
Key Responsibilities
Coordinate daily scheduling and delivery of care services
Support, guide and supervise Care Workers
Complete assessments and create person‑centred care plans
Set up and manage new care packages
Monitor service quality and compliance
Liaise with clients, families, NHS teams and Local Authorities
Contribute to service development and growth
Requirements
Experience as a Care Coordinator or Care Supervisor in domiciliary care
Strong organisational and communication skills
NVQ Level 3 in Health & Social Care (minimum)
Full UK driving licence and access to a vehicle
Commitment to delivering high‑quality, person‑centred care
