We are seeking an innovative and driven Advanced Pharmacist with a passion for Rheumatology and Dermatology to join our forward thinking and friendly team at HHFT.

Our department provides an environment that supports learning, team working and encourages individuals to develop within their role.

We are looking for someone with extensive clinical pharmacy knowledge and experience. You will have excellent skills in communications, negotiation and clinical pharmacy along with a strong team spirit and experience of supervising other staff.

The successful candidate will play a crucial role within the long term conditions to lead, develop and improve the pathways for patients. Your expertise will transform the approach to care and improve the outcomes for some of our most vulnerable patients.

You will support junior pharmacists and resident doctors working within the speciality in the review of complex patients and the sharing of your expertise to improve the care for our older frail adults. You will be using your prescribing and deprescribing skills to improve outcomes for our long term condition patients.

Deliver an advanced level of practice within the field of chronic conditions in Rheumatology and Dermatology.

Practice both independently and in partnership with the multidisciplinary team.

Use your prescribing and deprescribing skills to improve outcomes for our paitents.

Support junior pharmacists and resident doctors working within the speciality.

Be responsible for providing clinical leadership, supervision, and structured teaching for the multidisciplinary team to facilitate ongoing training and education.

Monitor and act on all reports of adverse incidents regarding medication that occur within the defined clinical area, ensuring necessary changes in practice, re-education of staff and sharing of the experience takes place within the Trust to help prevent a similar incidence from taking place.

Agree, set and ensure systems are in place to continuously monitor, review and reset clinical standards of care. That wherever possible clinical practice is research based and in line with nationally recognised best practice. Commenting on and creating clinical guidelines and procedures to support this.

*Candidatesmust be prepared to work towards an accredited Advanced Clinical Practitioner status to support the service, which will include a non-medical prescribing qualification*
