Summary

Responsibility for administering and supporting the SPoA process in the Community Support Team referral meeting. Work alongside the SPoA team in processing incoming referrals and signposting to clinical pathways in consultation with the lead clinician. Ensure timely input of referrals and timely completion of tasks relating to SPoA liaising with clinical operational leaders regarding issues arising regarding the process. Ensuring timely signposting and compliance with required waiting times. Accurate recording/registering of referrals via EPR and recording of outcomes. Communicating and acknowledging requests, advice and outcomes to referrers and others regarding clinical decisions relating to referrals. Manage Choose and Book e-referrals and liaise effectively with GP practices. Produce documents, charts, tables etc., by use of Microsoft IT packages e.g. Powerpoint, Word, Excel, Access and disseminate information via email. Taking of notes in a range of professional multi-disciplinary meetings as requested by nominated supervisor / manager.
