Summary


We are seeking a forward‑thinking, highly motivated pharmacist with expertise in frailty to co‑lead our established and progressive Frailty Team.

This post supports the NHS England ambition to shift care left, delivering acute‑level care through community and virtual models. The successful post holder will use advanced clinical pharmacy skills to support admission avoidance, early supported discharge and proactive frailty management. You will contribute to the design, evaluation and ongoing improvement of frailty and virtual ward pathways, aligned to Trust and national priorities around ageing well, flow, urgent and emergency care recovery and system integration.

Working within our Hospital at Home service, you will provide expert pharmaceutical care to patients managed safely at home, including participation in virtual multidisciplinary ward rounds, medicines reviews, prescribing within scope, and making clinical decisions supported by digital monitoring and escalation pathways.

Across community rehabilitation inpatient units and the community response team, you will provide senior clinical pharmacy leadership, supporting step‑down and step‑up care whilst ensuring safe, seamless transitions of care with strong communication across primary care and community services.

This is an ideal role for a pharmacist passionate about frailty and motivated to deliver high‑quality, patient‑centred care beyond the hospital walls.
• You will work under the Principal Pharmacist for Medicine (Acute & Frailty) with another Senior frailty pharmacist and co-lead a team of junior rotational pharmacists.
• Work collaboratively cross-site with the team at the Scarborough site.
• Deliver expert, patient-centred pharmaceutical care for frailty patients across acute, rehabilitation and community pathways
• Act as a senior clinical pharmacist within the Frailty MDT, providing specialist advice on complex prescribing and deprescribing decisions
• Lead on medicines optimisation, polypharmacy reduction and falls prevention initiatives
• Hold a non-medical prescribing certificate
• Participate in audit, research, and service evaluation to demonstrate outcomes and improve care
• Support and develop junior pharmacists, technicians, medicines management staff and the wider MDT
