Summary
The post holder will be responsible for the delivery of General Practice Nursing to the practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care. As an autonomous Practitioner, the Practice Nurse is responsible for the care delivered to patients, demonstrating critical thinking and skills in clinical decision-making in the management of Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery of a healthier Whitworth through listening and responding to Patients needs. The post holder may, in the future, be required to attend local Network meetings and support other developing activities in the area. Main Duties Hold reviews for long term conditions i.e. COPD, Asthma, Hypertension, CHD and Diabetes; Complete cytology screening; Monitor QOF; Delivery of vaccinations for both Adult and Children including Flu, Covid, Travel and B12; Attend MTD clinical meetings, LTI and monthly Practice meetings; Infection Control Lead for the Practice; Complete audits; Update Practice policies; Order stock i.e. vaccines, clinical consumables; Monitor emergency drugs and crash trollies. The post holder must be able to carry out all of the above and have up to date training for Cytology and Immunisations for both Adult and Children.
