JOB DESCRIPTION AND PERSON SPECIFICATION Job title: CARE COORDINATOR Line Manager: Digital & Operations Lead / Network Support & Transformation Lead Employed by and responsible to: Head of Workforce and Estates and Head of Finance and Performance at Kingthorne Group Practice. Clinical Director: Dr Mark Leighton Key Responsibilities: Develop a thorough understanding of the vaccination programmes and targets that all practices within the PCN are required to meet. Organise, plan and provide services to offer covid and flu vaccinations to the PCN and wider population either on site or remotely. To support practices in coordinating the vaccination of care home and housebound residents (including the preparation and coordination of patient lists, planning visits and communicating with patients) Using GP systems to identify the cohort of patients who require specific vaccination, create and monitor recall systems for these vaccinations and ensure patients are offered appropriate vaccines. Work in the reception area at your base to provide a service to patients attending PCN clinics and deal with queries from patients, health professionals and colleagues in person or via email, telephone and Systmone tasks. Use system reports alongside QoF, IIF & PCN DES requirements to proactively identify and work with specified groups of patients to deliver personalised care, to include the coordinating the booking of identified patients into the practice clinics. Take ownership and manage monthly rotas and appointments for Additional Roles Staff within the PCN also ensuring clinics are fully utilised and take action to book all available appointment Manage the network & Practice social media accounts to promote all health related content and PCN services. Record meeting minutes, capturing key discussions, decisions, and action points for timely distribution. Build relationships with key staff in practices within the PCN. Attend relevant meetings and integrate as part of the wider network team, providing information and feedback on relevant matters. Acting as the first point of contact for the clinical team to provide support with general queries. Actively be involved in Patient Participation Groups and feedback to the team on any actions. Help people to manage their needs through answering queries, making, and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care. Work alongside practice staff and PCN services to ensure cohesive care. Where appropriate assist people to access self-management health education courses, peer support or interventions that support them in their health and wellbeing. This may include attending any key meetings and liaising with hospitals and community services. Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people on to other health professionals within the PCN. Gather information and undertake enquires for the PCN and the wider group as and when necessary. Perform general administrative tasks as assigned by the PCN Manager to support PCN operations. Be a proactive team player, be able to manage their own time and workload and work under own initiative. Manage patient records with attention to accuracy, confidentiality, and data protection compliance. Working with the Management team to schedule and participate outreach events and forums. Support the coordination and delivery of the PCN led Multidisciplinary team (MDT) meetings. Attend PCN and Locality Cancer meetings to develop ways to increase patient attendance at screening appointments and role this out across the PCN. Dealing with patient registrations as and when requested by practices. Use system reports to send patient aimed feedback questionnaires following appointments both at practice and PCN level. File assigned pathology. Produce a quarterly newsletter to be distributed to practices and on social media. Action practice Smear prior notifications using the Care Identity dashboard. To plan notice boards across sites as required ensuring topics are seasonal and relevant. To work alongside practices to promote and increase NHS App utilisation. Facilitate PCN staff social events. You will be required to work across all PCN sites Personal/Professional development: Participate in any training programme implemented by the PCN as part of this employment. Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
