Summary
Preferred days/times:
Direct payment -7.5 hours a week
Self employed PA required
4 hours - Tuesday
2 hours - Thursday
3 hours - Sunday (every other week)
Female only due to personal care
Tasks/duties:
-To support with personal care tasks/toileting needs when needed.
-To escort the employer into the community to go swimming on Sundays (PA not required to get in the pool). On other days support to take part in hobbies, going out shopping and gardening.
-To support with household tasks.
PA to have a full, clean UK driving licence and will need to occasionally drive the Motability car.
Client main diagnosis:
Stroke causing left side weakness
Spasticity in left arm, hand and foot - calliper worn on left leg which often spasms.
Risk of falls some support may be required while out
Neurodiverse
Moving and Handling:
Poor mobility at risk of falls, unable to negotiate steps/kerbs/uneven surfaces.
Walks with quad stick within the home.
Uses folding stick or powered wheelchair when out in the community
Pets:
3 cats and fishes
Client enjoys:
Employer likes spending time with family, enjoys talking. Loves swimming, pets, house plants, cooking, the garden, and going shopping.
