Summary

Queen Victoria Memorial Hospital, Herne Bay is a supportive and collaborative place to work, where high‑quality patient care is delivered by a truly multidisciplinary team. Our inpatient service is led by a Matron and supported by registered nurses, healthcare assistants, therapeutic workers, physiotherapists, occupational therapists, rehabilitation assistants, care management staff, a pharmacist and our in‑house medical team. Together, we provide safe, compassionate and effective care for adults recovering from stroke and other complex conditions.

As part of Kent’s community hospital network, the unit plays a key role in delivering comprehensive healthcare for patients over 18. We work within an enablement model, helping individuals regain independence and return home as quickly and safely as possible. Patients are admitted from acute hospitals or referred directly from home via their GP or local community teams.

We’re looking for an organised, proactive and compassionate Discharge Coordinator to join our team at QVMH, Herne Bay. This is a key role, ensuring every patient has a safe, timely and well‑planned journey from admission through to discharge. As a Band 4 Discharge Coordinator, you’ll lead and coordinate effective discharge planning across the unit. You will drive daily board rounds, update expected discharge dates and actions, and ensure RIO board‑round notes are accurate. You’ll document discharge progress clearly in RIO, communicate with patients, families, external agencies and the multidisciplinary team, and work proactively with therapy, pharmacy, transport and community services to remove delays.

You’ll escalate any discharge barriers promptly to the Nurse in Charge, Matron, Flow Matron or Bed Manager, and work in line with local KPIs to support efficient patient flow and high‑quality care.
