Community Nursing Teams in Whitstable provide personalised nursing care to housebound adults with complex and long‑term conditions. Our aim is to help patients stay safe and well in their chosen environment, supporting independence and preventing avoidable hospital admissions. Working within teams aligned to Primary Care Networks, you’ll be part of an integrated multidisciplinary service focused on holistic, needs‑led care.

As a Band 5 Community Nurse, you’ll manage a varied caseload and deliver evidence‑based interventions that make a real difference to patients’ lives. Common interventions include palliative and end‑of‑life care, symptom control, advance care planning, medication administration including IV therapy, wound management such as leg ulcers and post‑operative wounds, bladder and bowel care, and health promotion to support long‑term condition management.

You’ll work closely with GPs, therapists, social care and wider community partners, contributing to coordinated, proactive care. You’ll have autonomy in planning care, time to develop your assessment and treatment skills, and access to training that supports advanced practice and builds confidence in managing complex caseloads.
