Summary
Self-employed PA required to support this individual to access the community and increase social involvement. This individual would like support to get out to their local area and further afield to go for walks, visit places such as coffee shops and other local shops. This support will be for 4 hours per week throughout the year.
The PA will also be required to assist with school runs during term times. 5 hours per week will be allocated for this support. This would be split into 30 minutes in the morning and 30 mins in the afternoon to walk the child to and from school.
Preferred days/times:
School run support - The PA would need to be at the individuals home address at 08:20 to support with walking the child to school for 08:40. The PA would need to be at the school for pick up at 15:20 and walk the child home.
The time that the 4 hours social support is provided can be discussed at interview.
Tasks/duties: accessing the community eg. going for walks/coffee/accessing shops in town. Getting into groups, getting out and about and low level exercise. To support with School runs during term time.
Client main diagnosis: Functional Neurological Disorder (FND) which can cause non-epileptic seizures and blackouts. Client recognises symptoms of the onset of a seizure, and those around them are able to recognise the symptoms of a blackout.
