Summary
Provide strategic leadership for pharmacy services across OCIT, Virtual Wards and community-facing pathways, ensuring delivery of safe, effective and patient-focused medicines optimisation. Lead the development and continuous improvement of clinical pharmacy services supporting admission avoidance, early supported discharge and care closer to home across UHCW NHS Trust. Collaborate with multidisciplinary teams and system partners across acute, community and primary care to strengthen medicines-related transfer of care and support integrated patient pathways. Act as a senior clinical pharmacy leader, providing highly specialised pharmaceutical advice in complex or high-risk clinical situations within OCIT and community services. Support the development and implementation of medicines governance frameworks, clinical guidelines and protocols to ensure safe and compliant medicines use across community-based services. Work with digital and operational teams to support systems that enable safe prescribing, medicines reconciliation and information sharing across care settings. Develop, deliver and monitor medicines optimisation initiatives that improve patient outcomes, reduce medicines-related harm and support efficient use of resources. Lead and support education, training and workforce development for pharmacy staff and the wider multidisciplinary team supporting OCIT and community services. Contribute to audit, service evaluation and quality improvement initiatives, using performance data to support service development and alignment with Trust and system priorities.
