Summary

Are you a passionate and dedicated qualified Social Worker with over 12 months experience in a statutory adults social care team who is looking to make a meaningful impact in the lives of older adults?

As a qualified Social Worker, you will;

Work with adults who have complex social care needs.
Manage a varied caseload with increasing complexity and autonomy.
Contribute to safeguarding, assessments, care planning, and service delivery.
Collaborate with health and care partners to deliver personalised services.
Promote independence and enable older people to live fulfilling lives using a strengths based approach
Work closely with service users, families, providers, and healthcare professionals to support safe, independent living in the community.
