Summary


The post holder is responsible, as part of a multidisciplinary clinical team, for delivering specialist nursing care to patients with diabetes, their carers and health professionals in secondary care.

The post holder is responsible for liaising with other HCP’s both specialist and non-specialist within the Trust and with colleagues in primary care to ensure patient safety both whilst an in-patient and on discharge.

The post holder will ensure that patients receive the necessary care and treatment to enable progress along the agreed pathways of care in line with the National Service Framework.
• To support and review patients admitted to the hospital with diabetes to ensure that their care and safety is maintained whilst they are an in-patient
• The post holder is responsible to manage a caseload of patients in secondary care
• The post holder works as an autonomous practitioner delivering knowledge and expertise within the specialist area
• To have good communication skills and work within the specialty team to ensure cohesive multidisciplinary management of patients
• To support and review patients admitted to the hospital with diabetes to ensure that their care and safety is maintained whilst they are an in-patient
