The Heart Failure Clinical Nurse Specialist will work as an independent practitioner to support heart failure patients in the community, by delivering high quality specialist nursing care to those admitted to the Rapid Diuresis Response/Virtual Ward (RDR/VW) caseload. The RDR Service sits within the ACERS Team and works alongside the community heart failure team, under the direction of the Heart Failure Clinical Nurse Specialist Team Lead and Cardiorespiratory Consultant Physiotherapist, with support from consultant cardiologists. The primary objective of the RDR/VW service is to reduce unplanned hospital admission for heart failure and to support people being discharged earlier from hospital.
The post holder will take a lead role in the day to day management of patients with acute decompensated heart failure predominantly in primary care, with some secondary care input. You will undertake assessments and reviews of patients in their home to formulate plans of care and titrate medication to treat their condition, improve symptoms and avoid hospital admission. You will work closely with GPs, cardiologists and the community heart failure team to deliver treatment plans and monitor patients’ response to treatment.
We are looking for an experienced, enthusiastic, pro-active, self-motivated nurse to join the team as one of the key links in the chain of gold standard care. Essential to this new role is a strong background in cardiology with relevant training and your excellent communication skills with which you will develop personal management plans and educate empower patients and carers for increased confidence in self-care. Experience of working in the community is desirable. This post will let your personal and clinical skills blossom as you set new ways of working in place to really make a difference.
