Summary

Support Worker

Newport | Up to £13.44 per hour | Days/Nights | Shift Work | Permanent

Introduction

Acorn by Synergie are recruiting dedicated and compassionate Support Workers to support an individual with mental health needs and a physical disability in their own home. This is a person-centred role focused on promoting independence, maintaining routines, and supporting safe engagement within the community.

Key Duties:
  • Deliver high-quality, person-centred care in line with individual support plans.

  • Monitor mental and physical health, emotional wellbeing, and overall presentation.

  • Support with all Activities of Daily Living (ADLs).

  • Promote independence and a structured, meaningful daily routine.

  • Encourage engagement in purposeful and enjoyable activities.

  • Support access to the community, appointments, and social opportunities.

  • Assist with faith-based routines and religious practices.

  • Safely drive the individual's vehicle where required.

  • Maintain accurate and up-to-date records in line with regulatory standards.

  • Uphold safeguarding responsibilities and maintain professional boundaries.

Requirements:
  • Registration with, or willingness to register with, Social Care Wales.

  • Enhanced DBS check.

  • Right to work in the UK.

  • Full UK driving licence.

  • Values-based approach with strong commitment to person-centred care.

  • Strong communication skills and ability to work as part of a consistent team.

  • Previous experience supporting individuals with mental health needs or physical disabilities (preferred).

  • Flexibility to work a range of shifts, including days, nights, and sleep-ins.

  • Willingness to complete all mandatory training.
