Summary
- Managing the 18 bed Hammersmith hospital cardio-geriatrics ward, together with a second geriatrician and providing Monday-Friday consultant ward rounds, discharge MDTs, complex discharge and family meetings, as well as clinical support to the resident team - MDT support across cardiac to support decision making and provide speciality input for complex cases - Liaison across other HH cardiac inpatient areas, providing frailty, complex-comorbidity, delirium, cognition and other geriatric syndrome assessments - Geriatrics and cardio-geriatrics outpatients - Leading the cardiac rehabilitation programme - Acute unselected take at St Marys hospital Full job description and person specification is provided.
