Summary
Assess and plan care for adults with disabilities: Conduct professional assessments, formulate care plans, and recommend specialist equipment or adaptations in line with legislation, COT/HCPC standards, and service policies.
Manage caseload and service delivery: Prioritise and coordinate interventions, equipment, and adaptations; maintain accurate records; manage risks and complex cases; liaise with colleagues, agencies, and carers.
Provide advice, training, and support: Offer specialist guidance to staff, service users, and carers; undertake moving and handling risk assessments; deliver appropriate training and equipment use.
Team development and leadership: Supervise and support staff, contribute to team performance, and assist in service improvement, performance management, and data collection.
Professional development and compliance: Maintain professional competence, adhere to Council policies (health & safety, Equal Opportunities, Customer Care), and demonstrate increasing autonomy and problem-solving skills for PO4 progression from PO3
