Summary

Key Areas of Leadership & Impact Clinical Leadership & Governance Act as Infection Prevention & Control (IPC) Lead Produce IPC annual statement & lead audits Lead on CQC matters within your remit Ensure robust SNOMED coding & chronic disease registers Chair nursing meetings (education, CQC, IPC) Long-Term Condition & Frailty Management Develop and embed care pathways for frail & housebound patients Oversee chronic disease clinics using evidence-based practice Identify and support patients at risk of long-term conditions Implement personalised care planning Health Promotion & Prevention Lead vaccination programmes (adult & childhood) Develop well woman & well man clinics Deliver opportunistic health promotion Coordinate travel medicine services Clinical Excellence Provide routine and complex nursing care Support minor surgery clinics Prescribe within PGDs and scope of practice Manage acute & chronic conditions Support safeguarding and vulnerable patient pathways People Leadership Lead nurse recruitment & retention Develop and mentor nursing team members Support training and professional development Foster a culture of excellence, empowerment and accountability
