The Role
In this specialist role, you’ll:
✨ Carry out person centred assessments in clients’ homes
✨ Recommend equipment and low/medium-level adaptations that make daily life safer
✨ Work closely with community services, Care & Repair teams, technical staff and housing partners
✨ Ensure timely, accurate case recording using internal and external systems
✨ Provide advice, support and signposting to clients and carers
✨ Champion safe, accessible homes across Rhondda Cynon Taf & Merthyr Tydfil
This is a role where your knowledge, empathy, and judgement directly shape the safety and wellbeing of the people we support.
What You’ll Bring
We’re looking for someone who is:
✔ TA Level 4 accredited or an Occupational Therapist (or equivalent)
✔ Experienced in assessing for small & medium aids and adaptations
✔ Skilled at working with older/disabled people
✔ A confident communicator and natural relationship-builder
✔ Highly organised, self-motivated and able to work independently
✔ Competent using Microsoft Office and case management systems
✔ In possession of a full driving licence and use of a car
Welsh language skills and experience of modern working practices are beneficial but not essential.
