Summary

We’re looking for a dedicated, resilient professional with a genuine interest in Mental Health and Acquired Brain Injury support to join our team as a Field Care Manager.
You’ll take ownership of care packages, lead and support staff teams, and ensure that every client receives safe, person-centred, and consistent care. This is a role for someone who is confident, organised, and ready to step into a position with real responsibility and impact.
What You’ll Be Doing
Completing robust risk assessments and developing clear, effective care plans tailored to individual needs.
Providing supervision, coaching, and appraisal to support workers.
Monitoring and reviewing support packages, including oversight of daily documentation.
Communicating confidently with clients, families, staff teams, and multi-disciplinary professionals.
Managing rotas to ensure safe and reliable service delivery.
