Summary

Stroke Support Coordinator

We are seeking an enthusiastic and compassionate Stroke Support Coordinator to support people rebuilding their lives after stroke.

Position: Stroke Support Coordinator
Salary: £25,623 per annum pro rata
Location: Home based with travel across South Gloucestershire and Bristol. Occasional attendance at the office in Hartcliffe, Bristol
Hours: 21 hours per week
Contract: Permanent, subject to 6 month probation
Annual Leave: 25 days plus bank holidays pro rata

About the Role

This is an exciting opportunity to join a long established local charity delivering Life After Stroke services as part of an integrated regional stroke service.

You will work as part of a multi-disciplinary team supporting stroke affected people, their partners, carers and families to adapt to life after stroke. The role is home based but involves visiting people in their own homes and working within the community.

Key responsibilities include:

- Providing confidential, person centred support to individuals and families affected by stroke

- Undertaking holistic assessments and screenings to identify needs and desired outcomes

- Delivering six month reviews for clients

- Offering flexible support including phone, email, home visits and virtual contact

- Signposting and guiding individuals through local services and systems

- Liaising with NHS colleagues and attending multi-disciplinary team meetings

- Facilitating and supporting groups including stroke cafés and communication workshops

- Supporting volunteers and contributing to service development

- Maintaining accurate client records

- Participating in fundraising events and training as required

You will need to work flexibly and must have access to a car.

About You

You will hold a professional health, social care or counselling qualification or have equivalent relevant experience.

You will also have:

- Experience supporting people within the health and social care sector

- Strong assessment skills and the ability to respond appropriately to complex needs

- Excellent interpersonal and communication skills

- The ability to work independently and as part of a team

- Experience managing a varied caseload and prioritising effectively

- Good IT skills including Microsoft Office

We would particularly welcome candidates with experience of stroke, Aphasia, group facilitation or supporting volunteers, although this is not essential.

Most importantly, you will be compassionate, empathetic, motivated and committed to helping people rebuild their confidence and independence after stroke.

You will be joining a close and committed team within a respected local charity dedicated to supporting people affected by stroke. The organisation works collaboratively with health partners to deliver high quality, person centred support across Bristol and South Gloucestershire.
