The Role
Are you passionate about making a real difference in the lives of children and young people with disabilities and their families?
We are looking for a skilled and committed Social Worker to join our Children with Disabilities Team.
In this vital role, you will work directly with children and young people who have a range of additional needs, along with their families, to ensure they receive the right support at the right time. You will complete holistic assessments, plan and coordinate services, and work collaboratively with families and partner agencies to promote positive outcomes and independence. Your work will focus on enabling children to thrive and reach their full potential, with their voice and experience at the centre of everything you do.
We are passionate about providing an inclusive, empowering, and person-centred service. We work closely with families to build trusting relationships, ensuring they feel supported, valued, and part of the process. Our commitment is to help children with disabilities live fulfilling lives, while ensuring their safety, wellbeing, and opportunities to participate in their communities.
Herefordshire is on a journey, and we would like you to be part of our next chapter. This is an exciting opportunity to have a genuine impact on the local authority and to help improve the lives of children and young people with disabilities across the county.
About You

• Skilled in assessment, planning, and coordinating multi-agency support, ensuring that each child’s individual needs are fully understood and met
• Committed to working collaboratively, inclusively, and creatively with children, families, and professionals from health, education, and social care
• Resilient, reflective, and confident in managing complex situations and making child-centred decisions
• Able to work effectively within a supportive and specialist team environment, contributing to a culture of learning and continuous improvement
• Registered with Social Work England and hold a recognised Social Work qualification (Degree/DipSW/CQSW) with a minimum of 3 years post qualifying experience
