Summary


Hawthorn is Intermediate Care Unit(HICU), its a Nurse Led Unit offering its patients Physical Rehabilitation and Step Down Facility to develop a safe holistic discharge plan.

HICU is a positive, enthusiastic working environment who strives on dedicated Team Work through working as party of a Multidisciplinary Team(MDT) of Nurses, Doctors, Geriatrician Consultant, Physiotherapists, Occupational Therapists, Pharmacist, Health Care Support Workers, Ward Manager & Ward Matron to deliver effective care to its patients.

The Health Care Support Worker(HCSW) will work closely with the Nurse who they assigned to per shift and take advice from them to help assist in the daily area of care which is provided to the patient.

The HCSW will be part of the Morning Nursing Handover, Direction of Care with the Nurse and the Discharge Planning of each patient through Nursing Handovers, Verbal Communication, Concise Note Documentation on System 1 and being part of the MDT.

• Effective Communication with the Nurses, Ward Manager, Ward Matron, Therapy Team & Doctors through Clinical Notes & Verbal Communication
• Working closely with the Nurse you are assigned with through your shift to deliver effective care
• Assessing the patients skin and reporting any skin damage(new or potential)
• Following the Patients Mobility Board which was written by the Therapy Team and advise of the patients improvements or decline
• Washing & Dressing the patients, always promoting independence where possible
• Toileting & Continence Care
• Catheter Care (Training Given)
• Stoma Care (Training Given)
• Venepuncture (Training Given)
• Bladder Scanning (Training Given)
