Summary
To provide a Care Coordinator resource for healthcare professionals and service users, working in collaboration with other members of the Multidisciplinary Team across the Primary Care Network (PCN). Working closely with healthcare professionals within the PCN to develop effective working relationships and identify and support a defined caseload of patients. Supporting the creation of personalised care and support plans which are patient centred. Providing coordination and navigation for patients and their carers across health and care services, plus other external agencies, so that their care plan is optimized and acted on. Providing clear communication to patients and their carers to ensure that people have good quality information to help them make choices about their care and the services the PCN offers. Using SystmOne, and other approved data collection resources, to obtain and utilise practice data support patient care and the delivery of services. Maintain a good working relationship with the practices within the PCN. Provide a comprehensive Clinic Coordinator resource across all PCN/Federation services and contracts, supporting the coordination and delivery of MDTs within PCNs. To ensure the effective running of all clinics. Support the Federation team with the mobilisation of new services.
