Summary
As a Discharge case manager you will be aligned to specific hospital in patient areas and will be responsible for a case load of patients at all levels of complexity to progress their hospital journey from admission to discharge
you will play a vital role within hospital flow and be the escalation point to progress a patients hospital journey
In liaison with ward staff community partners, social services and other healthcare colleagues facilitate timely, appropriate and safe discharges of patients
Ensure Early discharge notifications are completed and sent within 24 hours of admission
Ensure needs based assessments and any onward referrals are sent within a timely manner
Ensure all clinical systems are kept up to date throughout the day this includes cerner, optica and discharge dashboards
attend daily board rounds, MDT meetings Family and professional meetings
Be point of contact for patients and family members
