Summary
Deliver high-quality, person-centred care to patients under the direction and supervision of a registered nurse. Support assessment, planning, implementation and evaluation of individualised care plans. Undertake and record routine clinical observations and measurements, escalating concerns appropriately. Carry out delegated clinical procedures in line with training, competence and Trust policies. Support patients with activities of daily living, promoting dignity, independence and wellbeing. Accurately document care delivered and contribute to effective handover and communication within the multidisciplinary team. Recognise and respond to changes in a patients condition, escalating promptly to the registered nurse or appropriate professional. Support safe discharge planning by providing patient education and liaising with relevant services. Maintain infection prevention and control standards and contribute to a safe clinical environment. Participate in health promotion, patient education and support for carers where appropriate. Contribute to service improvement, audit and quality initiatives within the clinical area.
