Summary

Neway International are seeking Reablement Assistants to join our client, Liverpool City Council, based at Granby Aged Persons Home.

Duration: Initially for 6 months - likely to be extended.

Rates: PAYE: £14.93 per hour

About the Client
Liverpool City Council’s Granby Intermediate Care Hub is a 30‑bed reablement unit supporting individuals to regain independence following illness, injury or hospital discharge. The service works collaboratively with families, health professionals and partner agencies to deliver coordinated, person‑centred support that promotes independence and enhances daily living skills.

The Role
As a Reablement Assistant, you will support individuals to rebuild confidence, develop daily living skills and maximise independence. You will work as part of an integrated health and social care team, delivering high‑quality, person‑centred support in line with individual care and support plans.

This role operates on a three‑week rolling rota of early and late shifts, including weekends and some bank holidays:

Early: 07:30–15:00

Late: 14:30–22:00

An Adults DBS is required.

Key Responsibilities
Support the physical, social and emotional needs of service users in line with their care plans, promoting independence at all times.

Assist individuals with daily tasks as outlined in support plans, encouraging skill development and confidence building.

Work with service users to improve and maintain daily living skills, using appropriate equipment where required.

Collaborate with families, colleagues and external professionals to minimise risk and support individuals to remain in their own homes where appropriate.

Encourage and enable individuals to develop skills for interdependent or assisted living, including kitchen tasks, household routines, laundry and personal health management.

Contribute to the delivery of person‑centred support plans and participate in assessments and reviews.

Promote dignity, independence and empowerment, respecting cultural, spiritual and religious needs.

Maintain accurate records and report any significant changes in a service user’s condition or circumstances to the Line Manager.

Attend mandatory training and contribute to ongoing personal development.

Candidate Requirements
Experience supporting vulnerable adults in a care or reablement setting.

Ability to follow person‑centred support plans and promote independence.

Strong communication skills and the ability to work as part of an integrated team.

Reliable, compassionate and committed to high‑quality care delivery.

Willingness to work flexibly across a 7‑day rota.

Adults DBS (or willingness to obtain one).
