Summary

The ACP or ANP will work autonomously within their professional boundaries, providing high quality clinical assessment, diagnosis, treatment, and evaluation to patients presenting with both acute and long term health conditions. The post-holder will manage undifferentiated and undiagnosed presentations, making complex decisions based on advanced clinical knowledge and skills. Working as part of a supportive multidisciplinary team, the ACP or ANP will contribute to safe, effective, and patient centred care, supporting the practices commitment to high clinical standards and continuous service improvement. Regular debriefs and access to a supervising GP will be available throughout the working day. The main responsibilities of the role include: Clinical Care Deliver a high standard of patient care using advanced autonomous clinical skills and a broad, indepth theoretical knowledge base. Assess, diagnose, and treat patients presenting with a wide range of conditions, making independent clinical decisions and initiating direct referrals where appropriate. Manage acute illnesses, long term conditions, and minor ailments safely and effectively. As an independent prescriber, prescribe safe, effective, and appropriate medication in line with the current legislative framework. Triage & Urgent Care Work within the practices triage system to prioritise patient needs. Support urgent care provision by assessing and managing sameday or urgent presentations within scope of practice. Collaboration & Teamwork Work collaboratively with GPs, nurses, pharmacists, and wider healthcare professionals to ensure continuity and high quality patient care. Contribute to multi disciplinary discussions, clinical meetings, and shared care approaches. Health Promotion Promote health and wellbeing through health education, screening, lifestyle advice, and therapeutic communication. Support patients in self management of their conditions using evidence based strategies.
