Description
We are seeking a dedicated and compassionate ASW - Social Worker specializing in Mental Health, Learning Disabilities & Transitions at Grade 8 level to join our dynamic team. In this role, you will work closely with individuals experiencing mental health challenges and learning disabilities, providing support and interventions aimed at improving their well-being and quality of life. Your primary responsibilities will include assessing client needs, developing tailored care plans, and facilitating transition processes for individuals moving between services or stages in their life. You will collaborate with a multidisciplinary team, including health care professionals, educators, and family members, to ensure an integrated approach to care. Your expertise will be invaluable in identifying resources and support systems that clients can utilize, empowering them to achieve their personal goals and enhance their social inclusion. Responsibilities
Conduct assessments to identify the mental health and learning needs of clients.
Develop and implement personalized care plans to support clients through their transitions.
Facilitate communication between clients, families, and multidisciplinary team members.
Provide counseling and support to individuals and families dealing with mental health and learning disabilities.
Monitor client progress and adjust care plans as needed based on feedback and outcomes.
Advocate for clients’ rights and access to essential services and resources.
Educate clients and families about mental health and learning disabilities to promote understanding and awareness.
Requirements
A relevant degree in Social Work or a related field.
Current registration with the appropriate professional body or council.
Proven experience working with individuals with mental health conditions and learning disabilities.
Strong knowledge of relevant legislation and best practices in social work and mental health.
Excellent verbal and written communication skills.
Ability to work collaboratively with various stakeholders and team members.
Strong organizational and time management skills to handle multiple client cases effectively.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.21
