Summary

Purpose of Role:

To provide dedicated, continuous supervision and support to a patient identified as requiring 1-to-1 security due to risks such as aggression, absconding, self-harm, or vulnerability. The role ensures the safety of the patient, staff, and others within the hospital environment.
1. Continuous Supervision
• Maintain constant observation of the assigned patient to ensure safety and prevent incidents such as absconding, self-harm, or aggression.

Risk Prevention
• Identify and respond to potential risks or triggers.
• Intervene early to de-escalate situations using approved techniques.

Patient Escort
• Safely accompany the patient during movements within the hospital (e.g., to appointments, wards, or outdoor areas), ensuring secure and controlled transitions.

Incident Management
• Respond promptly to any incidents involving the patient.
• Support clinical staff during emergency situations or behavioural escalations.

Communication
• Maintain clear and professional communication with nursing and medical staff.
• Report any changes in patient behaviour, mood, or condition.

Documentation
• Accurately record observations, incidents, and interventions in line with hospital procedures.
• Complete handover notes for continuity of care.

Safeguarding & Confidentiality
• Adhere to safeguarding protocols and maintain patient confidentiality at all times.

Professional Conduct
• Maintain a calm, respectful, and non-judgmental approach.
• Uphold hospital values and standards of behaviour.

Compliance
• Follow all relevant policies, including infection control, health and safety, and restraint protocols.

1. Patient Supervision & Safety
• Maintain constant visual and physical proximity to the assigned patient.
• Monitor for signs of distress, agitation, or risk behaviors (e.g., self-harm, aggression, absconding).
• Intervene promptly and appropriately to prevent harm or escalation.
2. Escort Duties
• Safely escort the patient to and from clinical areas, diagnostic appointments, or outdoor spaces.
• Ensure secure transitions between departments while maintaining dignity and privacy.
3. Incident Response
• Act as first responder to behavioral incidents involving the patient.
• Support clinical staff in managing challenging behavior using approved de-escalation and restraint techniques if necessary.
• Complete incident reports accurately and promptly.
4. Communication & Collaboration
• Liaise regularly with nursing, medical, and security teams regarding patient status and risk factors.
• Participate in handovers and multidisciplinary briefings.
• Report any changes in patient behavior, mood, or condition.
5. Documentation
• Maintain clear and accurate records of observations, interventions, and incidents.
• Complete daily logs and contribute to risk assessments as required.
6. Professional Conduct
• Uphold a calm, respectful, and non-judgmental approach at all times.
• Maintain professional boundaries and adhere to hospital values.
• Wear appropriate uniform and identification.
7. Compliance & Safeguarding
• Follow all relevant hospital policies, including safeguarding, infection control, and health & safety.
• Ensure patient confidentiality and data protection standards are maintained.
8. Training & Development
• Attend mandatory training sessions (e.g., PMVA, MAPA, safeguarding).
