Summary
Main Responsibilities -Undertake administrative work in line with PCN directed priorities - Proactively identify and maintain current cohorts of patients to support their personalised healthcare requirements - Ensure regular and consistent communication with the clinical team regarding patient progress and any complications or guidance. Including liaising with secondary care and other health providers - Raise awareness of health promotion, run campaigns, to include seasonal priorities. - Coordinate clinics, liaising with the reception lead and contacting the identified patients with appointments - Manage patient initiated calls for help/signposting, ensuring patients are directed to the appropriate service (eg GP, Nurse, First Contact Physiotherapist, Pharmacist, Social Prescriber) - Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using SystmOne patient record system - Telephone contact to manage patients on the PCNs case load following any unplanned hospital admission (where appropriate) - Participate in MDT meetings to discuss patients from the PCN case load - Raise awareness of health promotion and screening such as NHS Health Checks and LD Health checks and coordinate invitations to ensure targets are achieved on a practice and PCN level - Assist and manage recall system for chronic diseases including acting as a point of contact for any queries regarding annual reviews - Acting as non clinical cancer Champion, coordinating referral practices, safety netting and screening activity in conjunction with the practice clinical cancer lead - Ongoing monitoring of PCN requirements including practice performance for IIF and QOF - Run audits and searches where necessary to identify patients for review - Monitor referrals to ensure tasks are completed and care delivered - Deliver reception services as needed by the Practice - Assist administrative colleagues with various tasks as required (for example - patient registration/deduction) - To carry out any other reasonable duties as requested by a manager to ensure quality of service for both the PCN and Practice
