Job Summary:
We are seeking a compassionate and skilled Support Worker to provide person-centered care to adults with complex needs. This role involves offering physical, emotional, and practical support to individuals who require specialised assistance due to disabilities, long-term health conditions, or additional care needs.
Key Responsibilities:
• Deliver high-quality care tailored to each individual's needs, promoting dignity and independence.
• Assist with personal care, mobility support, and daily living activities.
• Support individuals with medication management, health monitoring, and therapy sessions.
• Implement care plans designed by healthcare professionals, ensuring holistic support.
• Encourage social engagement, communication, and skill development.
• Provide emotional reassurance and companionship to enhance overall well-being.
• Use specialist equipment and techniques to support mobility and complex health conditions.
• Maintain accurate records, reporting changes or concerns to medical teams and families.
• Work collaboratively with families, therapists, and support networks to ensure continuity of care.
• Performing health related management to children with complex needs
Requirements:
• Experience working with individuals with complex care needs (preferred but not essential).
• Qualifications in Health & Social Care, Nursing, or related fields.
• Strong communication and interpersonal skills, with patience and empathy.
• Ability to work within a multidisciplinary team and follow individualized care plans.
• Understanding of safeguarding, infection control, and confidentiality policies.
• Practical knowledge of specialist care techniques such as PEG feeding, catheter care, or hoisting (training provided if needed).
