Summary

To be the Care Hubs first point of contact for all clinical effectiveness business,working in partnership with the Trusts Clinical Effectiveness and QualityImprovement Department. To report to Care Hub and Care Network on recommendations arising fromaudit, complaints, and incidents and ensure progress is made in respect to thoserecommendations and that learning can be evidenced as an outcome.AFC 8/11/23 JME-297-23 Head of Quality Band 8b To oversee the Care Hubs participation and reporting in all clinical effectivenesswork streams, including National clinical audit programmes, Nationalbenchmarking projects, local clinical audit programmes, NICE implementation,key performance indicators and quality improvement programmes. To work with the Trusts NICE Implementation Analyst to ensure all services inthe Care Hub appropriately use NICE guidelines and standards; including thefacilitation of meetings to coordinate completion of assessments, reviews andaction plans, using National and Trust developed tools and systems. To support the Care Hub to make effective use of national and local results andoutcomes falling from clinical effectiveness projects; e.g. Prescribing Observatoryfor Mental Health, Confidential Enquiries, National Clinical Audit, NationalBenchmarking studies, and clinical audit and quality improvement projects andinitiatives. To coordinate delivery and monitoring of all clinical effectiveness programmes theCare Hub participates in and ensure reporting and escalation meets therequirements of the Trust-led programmes. To build a relationship with the Clinical Effectiveness and Quality ImprovementLead to ensure the coordination and collation of data, information and examplesof good practice to showcase on behalf of the Borough for multiple reportingrequirements, including the Trusts Annual Quality Account. To actively identify best-fit person(s); i.e. subject matter experts, for involvementin the development and review of Trust developed clinical guidelines and Trustpolicies. To direct, manage and support staff who provide direct care to patients. This maybe as part of the senior management team, through on call responsibilities andthe support and guidance of the front line care teams when things go wrong eg.Grade 4 and 5 incidents. To be able to operate and access information from multiple information systemsdaily and to appraise and make recommendations To be frequently exposed to highly distressing and highly emotionalcircumstances These can include: meetings with families following suicides orunexpected deaths. Management of complaints with families when situations gowrong. To be readily available as a Senior representative at HM Coroners Court shouldconcerns arise and assurance be requested and/or to deliver the Trust approvedinvestigation report at coroners court. To support staff to complete Coroners statements and oversee the quality ofthese To support staff in attendance at Coroners Court. To be responsible for the whole care hub in relation to governance delivery to awide variety of services and disciplines within mental health care.
