Summary

As a Registered Mental Health Nurse within the Community Mental Health Service, you will: Clinical Practice Carry out comprehensive assessments, integrating psychosocial, risk and recovery based frameworks. Use therapeutic skills such as motivational interviewing, solution focused practice, and strengths based approaches to support engagement and recovery. Develop collaborative Recovery Plans and Risk Management Plans, ensuring these are shared with relevant professionals and agencies. Communicate highly complex and sensitive information with clarity and compassion to service users, carers and multidisciplinary colleagues. Work autonomously with support and mentorship from a supervising registered practitioner. Partnership Working & Communication Liaise closely with a broad range of internal and external partners including voluntary agencies, primary care, inpatient units, safeguarding teams, housing services and substance misuse services. Build strong relationships with carers and families, using a systemic approach where appropriate. Participate in multiagency meetings, professionals' meetings, discharge planning and case conferences. Professional Responsibilities Work in accordance with the NMC Code and all Trust policies, contributing to safe and high-quality practice. Engage in audit, quality improvement and service evaluation activities. Maintain accurate clinical documentation and uphold the highest standards of data protection and confidentiality. Leadership & Supervision Act as a positive role model, demonstrating professional behaviours aligned with Trust values. Provide supervision and appraisal to junior staff where appropriate. Support recruitment, induction and team development initiatives when required. Additional Requirements Ability to work flexibly and adapt to changing needs of the service. Ability to travel across Somerset to meet the needs of the role. Completion of Prevention and Management of Violence and Aggression (PMVA) training as required.
