Summary
Key Responsibilities You will: Lead digital transformation across the PCN, supporting practices to adopt modern, efficient, digitally enabled workflows Support implementation of NHS Englands Modern General Practice Model and neighbourhood health framework Strengthen multidisciplinary team working and support the development of Integrated Neighbourhood Teams Support practices to improve access, operational efficiency, and patient experience Lead transformation and improvement projects across the PCN Support practices to meet quality improvement requirements, including LQIS and other contractual or local priorities Promote consistency, best practice, and service improvement across all PCN practices Work with system partners including community services, mental health, local authority, and voluntary sector organisations Support the safe implementation of digital systems and improved use of data to inform care and service delivery
