Summary
START is a community mental health team located within The South London and Maudsley NHS Foundation Trust working with homeless people across Southwark, Lambeth, Croydon and Lewisham. The teams offer a holistic service for people with triple co-morbidity of need across mental health, substance use and physical ill health.
Key Responsibilities:
The post holder will:
• provide physical health assessment, interventions and referrals onwards to clients with acute and chronic physical ill health.
• work with Care Coordinators and consultant to support the holistic care of service users
The post holder will provide a physical health nursing service to clients under the care of the borough teams, by offering individualised assessment, direct personalised care and referrals onwards to specialist health services. This will be offered using an assertive outreach approach, seeing clients in community settings, as well as at team base clinics.

Professional / Clinical responsibilities
· Independently manage a group of patients within the specialty whilst working as part of the multidisciplinary team delivering specialised individualised and personalised direct patient care
· Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.
· Be competent to administer adults’ vaccinations.
· Refer to specialist services both statutory and non-statutory as needed
· Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action and to lead others to do the same.
· Ensure that patients receive high quality clinical care based on evidence-based practices
· Be wholly accountable for their practice in line with the NMC code of professional conduct and takes every reasonable opportunity to sustain and improve their knowledge and professional competence
· Ensure safe and effective care for complex patients requiring advanced specialist clinical skills and responsible for carrying out specialist nursing procedures to area.
· Ensure that sensitive patient records are recorded and stored appropriately.
· Ensure the cultural and spiritual needs of patients are recognised and accommodated within resources available.
· Participate in case conferences in relation to the management of patients, ensuring the coordination of ongoing care.
· Ensure staff takes precautions to minimise the risk of cross infection in relation to the safe disposal of body fluids (including blood).
· Facilitate a high quality, cost effective specialist service through monitoring and audit and other quality initiatives agreed by the Trust
· Lead on and participate in key developments relating to the specialty so that high quality patient focused services are promoted.
· Set, monitor and review standards of care on a regular basis and initiate corrective action plans
· Take an active role in risk assessment, supporting implementation of strategies to minimise risk. Ensuring incidents and near misses are reported, through promoting a no blame culture.
Information management
Keep accurate written records of assessment and treatment accordance with standards set and to input data into the electronic patient care notes system.
Education & Professional Development
· Have a good understanding of the health issues surrounding people experiencing homelessness and the Public Health strategies and issues relating to the client group (e.g. TB, blood borne viruses, and mental health)
· Provide specialist education to members of the clinical team, which facilitates learning and individual personal development.
· Develop the role according to patients changing needs and research findings, within the resources available.
