Summary


The successful person will be responsibility for the coordination of specialist, complex support of children/young people with a diagnosis of Muscular Dystrophy.

They will be a single point of contact for the children and young people and their family and also other health professionals involved, assisting with advice and navigation of the services available, coordinating access to ongoing assessments and services, and to ensure the patients care objectives are met in order to support patient choice.

This is an exciting opportunity to join the specialist team at Nottingham Children's Hospital and is suitable for a nurse or allied health professional therapist to work with this cohort of patients.

Be a single point of access regarding information and advice for those children and young people affected by neuromuscular conditions, their families/carers and the professionals working with them.

Work alongside the clinical nurse specialists/AHP’s to provide effective co-ordination of hospital and community care for children and young people with Muscular Dystrophy.

Meet families at the time of diagnosis, offering accessible information in an appropriate format, listening to their needs and signposting and referring to resources as required, in a time efficient way.

Be a single point of access regarding information and advice for those children and young people affected by neuromuscular conditions, their families/carers and the professionals working with them.

Work alongside the clinical nurse specialists/AHP’s to provide effective co-ordination of hospital and community care for children and young people with Muscular Dystrophy.

Autonomously manage their own paediatric caseload; co-ordinating, assessing and supporting children and young people and families/carers affected by Muscular Dystrophy.

Have oversight of children and young people admitted into hospital, encouraging and supporting early discharge planning by initiating the conversation early, during the assessment process.

Educate children and young people and their families/carers on all aspects of their specific disease and its progress/effect.

Provide training and information days for children and young people and their families/carers and other professional groups
