The West London Renal and Transplant Centre at Imperial College Healthcare NHS Trust (ICHT) opened at Hammersmith Hospital in November 2005 and is one of the largest facilities of its kind in Europe.
As a Band 5 nurse in the Renal department, you will collaborate with a diverse multidisciplinary team that includes specialist nurses, physicians, and allied health professionals. We are committed to helping you enhance your advanced assessment skills for managing highly complex and critically ill patients, thereby establishing a solid foundation for your nursing career.
By joining one of the largest renal and transplant centres in the UK, you will become an integral part of a team that is at the forefront of organ transplantation, making a significant impact on patients' lives.
You will have the opportunity to work with patients undergoing general nephrology care, as well as those in pre- and post-transplantation stages and receiving dialysis, in addition to individuals diagnosed with various autoimmune disorders. A significant number of our patients are admitted through our specialized emergency admission pathway, necessitating your provision of acute medical and surgical care. Throughout this process, you will receive comprehensive support aimed at enhancing your expertise in advanced assessment and patient management.
Previous experience in a renal setting is not a requirement. We welcome applications from those working in other specialties and newly qualified nurses.
• Carry out planned care for a group of patients without direct supervision
• Ensure the assessment, planning, implementation and evaluation of evidence based individualised patient care
• Assist and advise staff, both registered and unregistered in the delivery of patient care
• Maintain accurate clinical observations of the patient and act accordingly
• Undertake nursing procedures
• Have responsibility for the correct administration and custody of medicines according to Trust policy
• Monitor patients’ progress, work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on appropriately
• Act as the patient’s advocate
