Summary

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess Patient needs from a physiological and psychological perspective and plan clinical care accordingly. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice. Work with Patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimes, side-effects, and interactions. Prioritise health problems and intervene appropriately to assist the Patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support Patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. Support and manage health needs of women presenting for family planning, cervical cytology, or sexual health consultation. Assess, identify, and refer Patients presenting with mental health needs in accordance with national and local guidelines. Implement and participate in vaccination and immunisation programmes for both adults and children. Promote and deliver evidence-based care for patients presenting with aural conditions. Meet the needs of patients presenting for opportunistic wound care. As a senior Nurse you will be required to mentor/support/advise junior Nurses and Healthcare Assistants as required. Working with others: Work independently but also as part of a multi-disciplinary team to ensure the smooth running of the Practice The post-holder must recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members. Communicate effectively with patients and carers. Recognise peoples needs for alternative methods of communication and respond accordingly. Report any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the Nursing Team Demonstrate positive team development and effective networking. Demonstrate successful negotiation and influencing skills. Establish and maintain good liaison with other surgeries and agencies including secondary care. Leading by example: Be able to identify and implement systems to promote contribution and demonstrate the impact of advanced level nursing to the Health Care Team and the wider health and social care sector Act as a role model, be resilient and determined and demonstrate leadership in contexts that are unfamiliar, unpredictable, complex, and unstable. Provide leadership and specialist knowledge on therapeutic interventions, clinical practice, and service development. Personal Responsibilities: Act autonomously and independently Act within approved policies and clinical procedures Be innovative, highly skilled at assessing and managing risk and be consciously competent. Be responsible for decisions made and actions taken. Be a holistic practitioner, addressing nursing as well as medical needs - can see the whole person, combining clinical medicine with the art of caring, counselling, assessment, diagnosis, referral, and treatment. Use professional judgement in managing complex and unpredictable care and capture the learning from these experiences to improve Patient care and service delivery. Adhere to and work to professional code of conduct as regulated by the NMC. Maintain active registration with the NMC.
