Summary
Job Description Summary The core purpose of the role is to facilitate timely discharges, improve patient flow, and ensure a safe transition for patients leaving the hospital. Key Responsibilities Patient Assessment and Care Planning: Conducting comprehensive nursing assessments, developing, implementing, and evaluating evidence-based programmes of care. Discharge Coordination: Coordinating and facilitating patient transfers from wards/departments to the discharge lounge and their eventual destination (home, care facility, etc.). This includes liaising with social services, therapists, transport services, and pharmacy. Medication Management: Administering medications as needed and ensuring all discharge medications (TTOs) are received and understood by the patient/carer. Communication: Acting as the primary communication link for patients, families, and the wider multidisciplinary team, providing information and addressing concerns regarding the discharge process. Environment Management: Ensuring a safe and comfortable environment for patients in the lounge, which may operate during varying hours including weekends. Leadership/Supervision: Taking charge of the lounge when required and supervising junior staff, healthcare assistants, and student learners.
