Summary


Imperial College Healthcare NHS Trust seeks a motivated Band 7 Diabetes Specialist Practitioner to join our multidisciplinary team across five hospitals in Northwest London. Based mainly at Hammersmith Hospital, this full-time, on-site role involves delivering advanced clinical care, managing complex diabetes cases, leading service improvements, supporting staff, and providing education on medication and pathway development.

The post-holder will be required to cover any of the other sites in the event of an emergency though will be based at Hammersmith Hospital site. The post is required to provide a 5-day service from Monday to Friday on site.

Applicants should be enthusiastic, motivated and passionate about improving diabetes care for patients and should come with a minimum of 3 years post registration experience.

• The post holder may lead a team and be responsible for providing an advanced standard of care to patients at Imperial College NHS Healthcare Trust, which covers a multi-cultural and diverse population.
• The post holder will supervise the work of other more junior staff and support workers.
• The post holder may be rotational and will be expected to work across all sites of the Imperial College Healthcare NHS Trust.
• The post holder will assist the Clinical Lead in contributing to the planning, coordination, development and evaluation of the service. Together with other senior staff, you will act as a resource of specialist advice to the rest of the therapy service and other professions.
• The diabetes service at ICHT is a comprehensive acute service offering outpatient clinics specialising in:
-Ante-natal care
-Enhanced type 1 service for insulin pump therapy and continuous --glucose monitoring
-Structured education for type 1 diabetes
-Transition clinic for young adults
-Specialist foot unit
-Joint clinics for complex type 2 diabetes and obesity, liver and HIV
-Non-classical diabetes
-Diabetes and renal clinics
-Nurse-led clinics for young adults, type 1 diabetes and complex type 2 diabetes
• Post-holders will also need to support patients with diabetes on inpatient wards, Elective and non-elective admissions based on an e-referral system
• Links to community diabetes teams
• Patient /carer education
• Staff education and training
• Ward rounds
