Summary

The Role As a Primary Care Paramedic Practitioner, you will play a vital role in delivering acute care within the surgery and for our housebound patients in the community. At times it may require assessing the non-housebound who are unable to get to the surgery for reasons of new medical need. Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate. You will assess, diagnose, treat, refer or signpost patients who attend the surgery. The priority need is to support our home visiting requirements, frailty assessments, advanced care planning discussions (if suitable RESPECT planning) but the post holder can also hold their own clinics which would include a mix of telephone and face to face appointments. Key Responsibilities Provide clinical assessment, diagnosis, treatment, and evaluation of patients Manage same-day appointments, and home visits Work collaboratively with GPs, nurses, pharmacists, and other allied health professionals Support the management of long-term conditions, frailty, advanced care and RESPECT planning Promote health education and prevention To treat, as appropriate and within the scope of practice To signpost appropriately to other services To liaise with Supervising GP on a daily basis Assist in maintaining accurate data collection Effectively manage own time and workload Support and participate in shared learning across the practice What were looking for Forward thinking, proactive and committed Paramedic Practitioner with Primary Care experience Registered Paramedic with the HCPC Independent prescriber or willingness to work towards it (desirable) Excellent clinical, communication, and decision-making skills Ability to work independently and as part of a multi-disciplinary team Confident and efficient using Microsoft Office and IT based patient clinical systems EMIS is preferable Current DBS check Occupation Health Clearance Knowledge of Adult and Child Safeguarding procedures and referral pathways 18 hours a week What We Offer Supportive and inclusive working environment Regular GP led clinical supervision NHS pension scheme Flexible working arrangements Opportunities for career development in a progressive practice and PCN
