Summary
Key Responsibilities Support GPs, and other healthcare staff with medicines optimisation. Manage repeat prescription requests, ensuring timely processing and clinical checks. Conduct medicines reconciliation after hospital discharge or transfer of care. Support patients with medication queries, providing information and promoting safe use. Assist in audits, quality improvement, and prescribing safety initiatives. Support long-term condition management by optimising medication regimes under supervision. Liaise with community pharmacies, hospitals, and care homes to ensure continuity of care. Contribute to the practices prescribing budget by reducing waste and promoting cost-effective choices. Maintain accurate medication records in line with NHS and local policy. Ensure compliance with legislation, local policy, and national standards for medicines management.
