Description
We are seeking a dedicated and compassionate ASW - Social Worker with a minimum of 2 years of post-qualification experience to join our Independence & A team. Your expertise in social work will help you assess clients' needs, develop tailored support plans, and connect them with necessary resources. You will work collaboratively with multidisciplinary teams, local authorities, and partner organizations to deliver holistic care and advocacy for our clients. The ideal candidate will be experienced in dealing with diverse populations and will have a strong background in community development, safeguarding, and person-centered planning. Responsibilities
Conduct assessments of clients' needs and circumstances.
Develop and implement individualized support plans tailored to client needs.
Provide advocacy and support to clients navigating social services and community resources.
Work collaboratively with multidisciplinary teams to ensure a holistic approach to care.
Maintain accurate and up-to-date case records and documentation.
Facilitate workshops and support groups to promote client empowerment and independence.
Monitor clients' progress and adjust support plans as necessary.
Requirements
A recognized social work qualification and valid registration with the relevant professional body.
At least 2 years of post-qualification experience in a social work role.
Strong understanding of social work principles, ethics, and relevant legislation.
Excellent interpersonal and communication skills for effective client interaction.
Experience in case management and report writing.
Knowledge of community resources and services available to clients.
Ability to work independently and as part of a team, demonstrating strong initiative.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £23.73
