Summary

Focus Birmingham is a local charity supporting and empowering individuals with visual impairment and complex needs to live fulfilling, independent lives.

We are currently seeking a compassionate, organised, and proactive Transition and Placement Coordinator to support adults as they move into our specialist Day Service.

About the Role
In this rewarding role, you will:
Support individuals through a smooth and positive transition into our Day Service
Work closely with families, carers, and external agencies
Coordinate placements and ensure individual needs are understood and met
Collaborate with a multidisciplinary team to help every individual thrive

About You
If you are:
Caring, patient, and person-centred
Highly organised with strong communication skills
Confident liaising with families and professionals
Passionate about supporting people with complex needs and visual impairment
Want to work within a dedicated and compassionate team
