Crynodeb
We are a committed and passionate team of geriatricians looking to expand our services further across the hospital and into the community to enable more older people to benefit from specialist geriatric assessment. We provide acute in-patient services on an elderly care ward, ortho-geriatric services, front door frailty assessment, a virtual ward frailty pathway and out-patient clinics at the West Suffolk Hospital site. We also oversee care of intermediate care beds in the community hospital at Newmarket. We believe in working flexibly and supporting each other to meet service needs and the personal development needs of our staff. Our priorities for the next 2 years include the establishment of our frailty hub model a comprehensive acute frailty assessment pathway to offer same day advice & assessment to community clinicians, expansion of our virtual ward pathway and using new technology to provide remote assessment of patients in care homes. At the same time we wish to remain focussed on providing high quality care to our inpatients and their families and excellent training opportunities & support to our trainees.
