THE POPLARS MEDICAL CENTRE JOB DESCRIPTION Job Title: GP Lead Practice Pharmacist Location: Poplars Medical Centre, Swinton Position: Full-time, Permanent Salary: Depending on experience benchmark NHS Band 8a8b: £55,000£74,000) Patient List Size: Approximately 11,500 patients PRACTICE OVERVIEW Poplars Medical Centre is a friendly, supportive GP practice serving around 11,500 patients in Swinton. JOB SUMMARY The post holder will work as the Lead Practice Pharmacist, providing advanced clinical pharmacy services within primary care. The role combines direct patient care, prescribing leadership, medicines optimisation, governance oversight and team leadership. CLINICAL DUTIES Deliver structured medication reviews for patients with complex polypharmacy, frailty, multimorbidity and high-risk prescribing. Provide independent prescribing within scope of competence for acute and long-term conditions. Support safe and effective repeat prescribing, including acute requests, synchronisation and streamlining. Work collaboratively with GPs, nurses, advanced practitioners, pharmacy technicians and the wider MDT. MEDICINES OPTIMISATION AND PRESCRIBING LEADERSHIP Lead prescribing quality improvement initiatives aligned to QOF, ICB priorities and practice objectives. Analyse prescribing data to identify trends, risks and opportunities. Implement NICE guidance, MHRA alerts, shared care protocols and local policies. Promote safe deprescribing and shared decision-making. Strengthen antimicrobial stewardship. GOVERNANCE, RISK AND SAFETY Lead medicines-related governance, including audits, incident review and learning events. Oversee high-risk drug monitoring and shared care compliance. Maintain systems for repeat prescribing, controlled drugs and prescribing safety. Support CQC compliance. TEAM LEADERSHIP AND SUPERVISION Provide day-to-day leadership of pharmacy staff and prescribing clerks including line management responsibilities Deliver supervision, mentorship and competency assessment. Support training and induction of ARRS and other clinicians. DIGITAL, DATA AND SYSTEMS Use EMIS, Ardens, AccuRx and prescribing analytics tools. Support electronic repeat dispensing and digital innovations. Review and improve prescribing workflows.
