Summary
To work as part of a team which provides care and treatment to individuals with mental ill health. To provide direct care according to the needs of the service users and within the care programme approach/care management framework. To provide support, give time to an allocated group of service users and, therefore, promote recovery, assisting in maintaining the links with their community environment. To assist the Care Co-ordinator /key nurse in the planning, implementation and evaluation of individual care plans.
