You will be able to find a full Job description and Person Specification attached within the supporting documents or please click Apply now to view in Trac To lead and facilitate on the strategic implementation of the Children and Young Peoples Continuing Care process within the Community Childrens Nursing Service, ensuring effective, efficient and appropriate high quality care is provided, monitored and evaluated. To support the delivery and development of improved performance across all children and young peoples continuing care processes, using an outward looking approach, attending and participating in local and national professional forums. To provide expert knowledge, strong leadership and direction to enable the Community Childrens Nursing Team to implement the Children and Young Peoples Continuing Care Guidance in partnership with other agencies and in line with national policies. To work with other professionals and partner agencies to actively contribute to the implementation of local and national policy and guidelines for children and young people with complex needs, thus ensuring a joint health and social care approach to assessment, care planning and review. To lead and support Health Assessors to make recommendations with regard to funding, placement and level of care and be able to effectively demonstrate outcomes and oversee the evaluation of the process and ensure equity across the Gwent area. To lead and manage a quality assurance process for children and young peoples continuing care, ensuring standards are maintained and risk is managed appropriately. To work collaboratively with other professionals and partner agencies to support appeals and complaints processes, ensuring these are dealt with fairly, openly and transparently. To work autonomously, manage own time and scope of professional practice To lead in partnership with adult practitioners on the transition to adult services of children and young people with complex healthcare needs, providing advice, support and guidance to young people, their families and key staff. To demonstrate sound knowledge of the Adult CHC process and attend planning meetings, training as necessary. To ensure Children and young People who have agreement for continuing care are entered onto the National complex Care Database. To take responsibility of audit in relation to children and young peoples continuing care and present reports to Health Board senior management team; this includes the maintenance of an accurate database of continuing care cases with record of financial spend and resource requirements. To present continuing care cases to the Health Board Multi-agency Decision Making Panel using professional judgement and clinical expertise to support recommendations for care whilst demonstrating sound knowledge of resource restraints To make recommendations on workforce and skill mix requirements in relation to assessed need and in line with prudent healthcare agenda To work with other professionals to ensure the provision of equipment for children and young people who meet eligibility for children and young peoples continuing care To calculate the financial cost of individual continuing care packages for children and young people and monitor the financial impact on the community childrens nursing service To work with team managers across the service to ensure accredited training programmes for HCSW are implemented, monitored and reviewed To be responsible for ensuring the safety and welfare of patients and staff ensuring that they observe and implement Health and Safety Policies identifying, and achieving training and developmental needs through the implementation of Health Board policies and procedures. To participate in meeting the needs of a 24 hour, 7 days a week Service and work in partnership to meet on-call Service needs to minimise the need for hospital admissions To deputise for Senior Nurse and support them in the development and management of the Community Childrens Service
